I declare that my son/daughter does not suffer from any disease, illness or condition that prevents them from performing the physical/sports activities and exercise for which I am enrolling them, and I expressly renounce any demand for liability for any lesion that may occur as a consequence of the habitual practice of the activities that are characteristic of the tennis school.
In the case of a student who suffers from a disease, illness, condition or lesion (congenital or acquired) this enrolment form must be accompanied by a medical certificate stating the minor’s ability to carry out normal sports activities and/or if applicable the medical guidelines for adaptation of said activity to the student’s physical condition.
I give my authorization for participation in the activity during the course of 2021-22 and I declare that I have read and accept the school’s and the sports centre’s rules that are posted on the information board in the centre’s reception area.
